You are a good candidate for dental implants if you have jawbone ridges that are in good shape and your are in good physical health. The dental implant, forms chemical and mechanical bond, and the bone actually grows into the implant forming an osseointegration between the bone and the implant. For this type of prosthesis I discuss with the patient their concerns regarding existing dentures or dental situation, so I can help guide the patient toward their goals, with existing treatment modalities which would achieve their goals. Generally we are talking about implant supported bridge or hybrid denture although there may be other treatment options including saving teeth, intravenous or oral sedation, etc. Sometimes new denture may be needed to guide us toward the final fixed prosthesis to give the patient and myself a visual reference, especially if the existing denture(s) are too worn or existing teeth are not useful as a visual guide. At about that same time we do impressions and 3D imaging of the patient’s mouth to visualize the anatomy, volume and location of bone for implant placement. With the images/scans and models of the existing teeth and/or dentures we can virtually simulate the patients mouth.
Photo 1 shows where the patient’s lower teeth were which are leaning too far forward; Photo 2 shows the virtual removal of the lower teeth and new placement of where we repositioned virtual teeth on the upper and lower arches. This dramatically improves her profile and esthetics. After the teeth are virtually positioned, the surgical guides can be made to assist in placement of the implants for accuracy.
Then the actual implants are placed. Immediately dentures could be attached to the implants for same day teeth also called teeth-in-a-day. Generally though the patient will continue to wear their dentures with the implants under the tissue for 3 months while the implants integrate or attach the bone.
After the 3 months the implants are uncovered if needed. Then an indexed impression to relate all the implants to the ridge is made to fabricate the Fixed bridge or hybrid denture as shown.
